Сравнительный анализ результатов лапароскопической предбрюшиннной пластики паховых грыж и герниопластики по Лихтенштейну

Цель исследования – сравнительный анализ результатов трансабдоминальной предбрюшинной лапароскопической пластики и паховой герниопластики по Лихтенштейну. В Клинике № 2 Центральной городской клинической больнице Великого Новгорода в период с 2017 по 2023 годы оперировано 695 больных с паховыми грыжами из них у 322 (46,3%) хирургическое вмешательство выполнено эндовидеохирургическим методом и 373 (63,7%) оперировано открытым методом по Лихтенштейну.

При лапароскопической герниопластике конверсия на открытый доступ произведена у 4 (1,2%) больных. Сравнительный анализ результатов показывает, что лапароскопическая предбрюшинная пластика паховых грыж имеет определенные преимущества перед открытыми вмешательствами, в частности отмечается статистически достоверное снижение времени оперативного вмешательства (54,7 мин и 63,8 мин соответственно), сокращение периода купирования послеоперационного болевого синдрома и назначения анальгетиков (1,3 дней и 2,6 дней), уменьшение числа послеоперационных осложнений (5,0 % и 8,6 %) и сроков пребывания в стационаре (3,6 дней и 5,2 дней соответственно). Лапароскопическая предбрюшинная герниопластика, если проведена со строгим применением стандартизированной техники и опытными руками, является отличным подходом для лечения паховой грыжи по сравнению с пластикой по Лихтенштейну, независимо применялась ли она при односторонних или двусторонних, первичных или рецидивирующих паховых грыжа и характеризуется меньшим количеством послеоперационных осложнений и рецидивов со всеми преимуществами лапароскопической хирургии, такими как уменьшение послеоперационной боли, ранняя активизация и выписка из стационара.
