В статье рассматриваются вопросы онкопатологии детского возраста. На основании данных литературы анализируется структура онкологических заболеваний у детей в Российской Федерации и в Новгородской области. Показано, что в виду редкости онкологических заболеваний в детском возрасте, выявляемость их на этапе амбулаторно-поликлинического звена требует активной проработки с педиатрами, за счёт повышения осведомленности персонала о данной группе заболеваний и формирования в рутинной работе навыка онконастороженности врача. В статье освещены механизмы патогенеза редкого злокачественного эмбрионального новообразования почки — нефробластомы (опухоли Вильмса), описана клиническая симптоматика. Приведено собственное наблюдение авторов ребёнка грудного возраста с диагнозом опухоль Вильмса. Получены выводы о течении заболевания у ребёнка, сроках манифестации клинической картины. Наблюдение демонстрирует актуальность своевременного проведения скринингового ультразвукового исследования брюшной полости и почек детям первого года жизни, необходимость повышения уровня знаний в вопросах детской онкологии педиатров амбулаторно-поликлинического звена, специалистов ультразвуковой диагностики, поскольку своевременная диагностика и раннее лечение значительно повышает вероятность достижения ремиссии злокачественных заболеваний у детей.

The article deals with the issues of childhood oncopathology. Based on the literature data, the structure of oncological diseases in children in the Russian Federation and in the Novgorod region has been analyzed. It is shown that due to the rarity of oncological diseases in childhood, their detectability in outpatients in clinics requires active study with pediatricians by increasing staff awareness of this group of diseases and developing doctors’ oncological alertness skill in routine work. The article highlights the mechanisms of pathogenesis of a rare malignant embryonic neoplasm of the kidney — nephroblastoma (Wilms tumor), describes the clinical symptoms. The authors' own observation of an infant with a diagnosis of Wilms tumor is given. Conclusions were obtained about the course of the disease, the timing of the manifestation of the clinical picture. The observation demonstrates the relevance of timely screening ultrasound examination of the abdominal cavity and kidneys for children of the first year of life, the need to increase the level of knowledge in pediatric oncology of pediatricians of outpatient clinics, ultrasound diagnostics specialists, since timely diagnosis and early treatment significantly increase the likelihood of achieving remission of malignant diseases in children.
